What is Prior Authorization and how does it work when accessing healthcare?

As the country continues to struggle with how to cope with rising overall health care costs, the increased use of pre-approval has become a powerful tool for insurers.

How does it work and why is it used? One of the common themes in the healthcare industry is service overuse. When an insurance company refuses to pay or reduces a payment, this is sometimes called “medically non-essential services.” This can be confusing for patients with health problems who seek medical attention. Physicians believe that diagnostic work is required to confirm a diagnosis, after which insurance companies deny preapproval requests. In other words, you are effectively refusing to pay for the requested service. A “pre-approval” process is the most common definition of an insurance plan’s process to approve only “high-cost care.” How disruptive would that be for the patient? Doctors believe it is necessary. Tests are available on the company’s network. But the insurance plan says “no”. Is this “insurance-led care or physician-led care?”

Pre-approval is used to determine whether laboratory drugs, chemotherapy drugs, hospitalizations, or nearly any service ordered meets the insurance plan’s definition of “value-based or medically necessary care.” A tool used by insurance plans. Pre-approval is in addition to ongoing dialogue with the insurance plan to “negotiate” coverage, change to another test, and communicate the patient’s ability to pay. It can be of concern to the medical community as it can cause significant delays in treating the condition. Patients often hear from healthcare providers that they’re asking for an MRI to confirm damage, but need to wait to see if insurance covers it. Or, “I need a vitamin D test to rule out an unexplained muscle pain problem, but I know insurance won’t cover it.” Is my doctor ordering something “not medically necessary”? ’ Wow, healthcare terminology can be confusing.

idea: When your health requires medical tests, procedures, or special medications, you are the one who worries the most. Your healthcare provider is ready to provide your insurance plan with the necessary medical information during the pre-approval process. But if your insurance plan denies care, call your insurance plan right away. The card usually has a customer service number on it. This is the first call as it takes a little “push” to reach the right person to discuss the denial. Request to know why it was not approved. What can you do to get approval? What other options are there? Patients are the most affected and the least capable of understanding what has happened. Controlling the cost and inappropriate use of medical resources is the driving factor behind pre-approval of services. Patients are also the ones who pay for their insurance premiums and out-of-pocket costs after receiving services, so a balance is important to ensure they receive the most appropriate and timely care. The country is moving towards ‘value-based care’ and has a long way to go to establish a definition of it, rather than just value-based payments. ’ We won’t be bored, but let’s hope that through supportive education it becomes a little less frustrating.
