initiative, Known as CalAIMprovides social services to some of Medi-Cal’s most ill and costly patients, including: home delivery healthy mealssubsidies for moving-in expenses, and realization of a safe living environment by repairing houses person with asthma.
But, as Hart pointed out, the reforms are leaving many patients behind. Mr. Hart spent time at his MLK Community Hospital in South Los Angeles. There, she told her patients, health consumer advocates, and hospital executives hadn’t improved care for the vast majority of patients who weren’t receiving new social services. They also found that low reimbursement rates for doctors and other providers have created a “personalized and unequal” system of care in communities with disproportionately high rates of devastating chronic diseases such as diabetes, liver disease and hypertension.
