Aledade and Humana have expanded their value-based care partnership to support rural health clinics and federal qualified health centers (FQHCs). announcement Thursday.
Based in Bethesda, Maryland, Aledade helps independent practices, health centers and clinics engage in value-based care through data insights, policy expertise and contract support. In 2023, Humana launched a 10-year partnership with Humana, and Humana’s Medicare Advantage members can receive primary care from Aledade Physians using value-based arrangements.
According to the announcement, the expansion of the partnership will help Aledade FQHC and Rural Health Clinic Partners share Medicare Advantation savings. Aledade and Humana provide early resources and support to help you succeed in value-based care. The expansion will support FQHCs and rural clinics in 26 states.
“This is the latest step in our longstanding collaboration with Humana, addressing the unique and evolving needs of our health centre partners,” announced Dr. Farzad Mostashari, co-founder and CEO of Aledade. “It demonstrates our commitment to addressing health centers’ disparities, recognizing the challenges health centers must address in order to provide high-quality preventative care to a highly-needed population.

Humana Insurance President George Renaudin added that companies have a shared mission in expanding value-based care.
“We are pleased to strengthen our relationship with Aledade, who shares its strong commitment to value-based care and its proven positive impact on health outcomes,” Renaudin said in a statement. “This expanded agreement is a critical development as it can improve access to quality healthcare in areas such as rural communities across the country, where hospitals and clinics are historically scarce.”
Aledade collaborates with over 300 health centers across the country and “work across diverse rural, suburban and urban care environments, as well as primary care organizations of all sizes.” Approximately two-thirds of these health centers are in the area of shortage of health professionals.
Aledade is not the only company where clinics and health centers support value-based care. Others include Astrana Health and Agilon Health.
